Orlando, Florida Nursing Home Safety Issues

The state of Florida and the federal government regularly update their long-term care database system with detailed information on all deficiencies, referrals and violations.

According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars for health inspections, four out of five stars for personnel issues, and three out of five stars for quality measures.

State investigators found the nursing home “failed to provide physician-ordered wound care for a sacrum pressure ulcer.” On August 13, 2018, investigators interviewed the department manager at the West Wing outside a resident’s room.

The department head stated that “the resident had an undetectable pressure ulcer on the sacrum, a stage II pressure ulcer on the left heel, and a blood blister on the right heel.” She stated that he could not even be interviewed with an interpreter. [The resident] was observed lying on his back in bed.”

Inspectors observed wound care on August 15, 2018, from 8:55 a.m. to 9:40 p.m. A registered nurse (RN) and a registered nurse were present. The resident who needed a Spanish interpreter was “non-verbal but occasionally opened his eyes during wound care.”

The experts asked the head of the department and the registered nurse to check the doctor’s instructions. The nurse “stated that she checked the order and simply forgot to apply the wet Santyl gauze to the wound bed or skin around the wound.” The department manager told the nurse that she “had a copy of the actual order that should have been brought into the wound care room to ensure that the wound care ordered by the physician was performed correctly.”

Failure to Provide and Implement an Infection Prevention and Control Program

“The facility failed to ensure that isolation guidelines for [a resident] were followed with respect to contact isolation to prevent cross-contamination and spread of a contagious infectious disease in the East Wing.”
